Completed statements are addressed to the Missouri Secretary of State's Corporations Division, and the fee is ten dollars.
What the form wants is short. Your entity name as it appears on the state's records, whoever is stepping in, and the Missouri registered office address, which has to be a genuine street address here. A post office box qualifies only when it rides alongside that street address, never on its own.
Then the instruction that governs your calendar: the new agent must sign the statement, or written consent to the appointment must be attached. That rule exists so nobody is enrolled into the job without agreeing to it, and it has a useful side effect for you. Because the successor's signature is required, the successor has to exist first, which makes the correct order impossible to get wrong once you know it.
